Sesamoiditis & Ball-of-Foot Pain

OPD

What Is Sesamoiditis?

Sesamoiditis is an inflammatory condition affecting the sesamoids—two tiny, pea-sized bones embedded within the tendons beneath the big toe joint. Acting as pulleys for the big toe, these bones can become inflamed, fractured, or irritated due to repetitive impact, high arches, or excessive weight-bearing on the forefoot.

Common Symptoms

Dull, lingering pain beneath the big toe joint on the ball of the foot · Swelling and bruising over the base of the big toe · Sharp pain when bending or pushing off the big toe while walking · Difficulty putting weight on the inner ball of the foot

Our Treatment Approach

Forefoot pressure analysis & digital X-rays to check for sesamoid stress fractures → non-surgical offloading (custom orthotics with a dancer’s pad or sesamoid cutout) → targeted anti-inflammatory measures & immobilization → progressive rehabilitation for activity return.

Frequently Asked Questions

How do custom insoles help sesamoiditis?
Custom insoles with a sesamoid cutout transfer weight away from the inflamed sesamoid bones to surrounding foot areas, letting the tissue rest and recover while you walk.
Can sesamoiditis cause big toe stiffness?
Yes, persistent inflammation around the sesamoid bones can restrict the tendon's sliding mechanism, causing painful stiffness in the big toe joint.
